About this Survey

This questionnaire was originally developed for research into the role of nutrition in health and disese.

Hundreds of research groups are now using it, with many published scientific reports.

Studies have shown it to be comparable to detailed diet records in estimating average nutritional intake.

NASA is using it as a baseline measure of the nutritional status of our astronauts on the International Space Station, to study the effects of long-term space flight.

    Do not include eggs used in cooking, such as in cakes and custards.

    BE CAREFUL NOT TO DOUBLE-COUNT. In the how often section, tell us how many days you eat eggs. Then, in the how many section, tell us how many eggs you usually eat on those days.

    If only egg whites are eaten, do not count these. In answering the how many question, just count the number of yolks. For example, if you usually scramble one egg yolk and two egg whites, your answer would be one.
